- Employment Application
And Interview Form
- Students new to the work-study program must complete a Student
Employment Application, and return it to The Office of Student
Employment, Campus Box 44. Or to the 2nd Floor of Norrington, in the Student Financial Services Office.

- Transfer Request Form
- The Transfer Request Form will be submitted by the student to
his/her current supervisor for authorization to transfer. The student
will also submit this form to his/her new supervisor for signature
before any work can begin.

Requisition Form
- The Student Requisition Form is used by the Department
Supervisor to advise Student Employment of their need for Work Study
Employees. It is completed and submitted to the Student Employment
Coordinator Samuel Taibi, at Campus Box 44. This form is used
to determine how many student workers are required within your
department. Questions regarding this form or its use may be
directed to ext. 6298.

- Warning Form
- The Disciplinary Warning Form must be completed and submitted
to the Student Employment Office to be placed in the student personnel
file. Call Samuel Taibi with any questions regarding this
matter at (816)584-6298.

- Termination Form
- The Termination Form must be completed by the supervisor and
returned to the Student Employment Office. For information

Evaluation Form
- The Student Employment Evaluation Form will be used to evaluate
tour departments Student Employee's at least once per semester.
