Forms

For reviews of research with human participants, the forms should be submitted to dennis.kerkman@park.edu.

For reviews of research with animal participants, the forms should be submitted to douglas.burns@park.edu.

If your research meets the definition for "exempt", then you need to submit

1. An IRB Application (Login to PirateNet required)

If your research meets the definition for "expedited review", then you need to submit

1. An IRB Application (Login to PirateNet required)
2. A letter of informed consent
3. Copies of all questionnaires, interview protocols, etc.

These must be submitted by the second Wednesday of the month (one week prior to the monthly meeting of the IRB).  No IRB applications will be reviewed during the last month of the Fall or Spring Semesters of the Park University Home Campus Day Program.    DO NOT BEGIN TO COLLECT DATA UNTIL AFTER YOU HAVE RECEIVED WRITTEN APPROVAL FROM THE IRB.

If your research meets the definition for Full Board review, then you need to submit five (5) copies of the following:

1. An IRB Application (Login to PirateNet required)
2. A letter of informed consent
3. Copies of all questionnaires, interview protocols, etc.

These must be submitted by the second week of the month (one week prior to the monthly meeting of the IRB).  DO NOT BEGIN TO COLLECT DATA UNTIL AFTER YOU HAVE RECEIVED WRITTEN APPROVAL FROM THE IRB.
