• Insurance Waiver

           -Required for any residential, F-1 international, nursing,

            or student athlete who is showing proof of your own

            medical insurance coverage (F-1 international students

            must also provide proof of repatriation coverage)

• Insurance Enrollment

          -Required for any student who is residential, F-1

           international, nursing, or student athlete and does not

           have proof of medical insurance. It is optional for any

           other student on
