• Declaration of Major

           - Used to declare a major, change a major or add

             anything to your degree program (i.e. second major,

             minor (s))

• Authorization for Release of Information (FERPA)

          - This form must be filled out if you want university officials

            to be able to discuss any of your directory information

            with any other person (family members, parents, friends,

            partners, etc.)
